How to enroll a new student         1) Download, complete, and submit the online enrollment request. (After submitting the request, a BPS representative will contact you to assist you in completing the enrollment process.)     • Enrollment request form for 11th and 12th grade students     • Enrollment request form for 9th and 10th grade students     • Enrollment request form for all other students   2) Gather required information and complete forms to submit during enrollment appointment.     • Social Security Number     • Birth certificate     • If student is living with a guardian or custodial parent, notarized proof of guardianship or a signed document from the courts is required.     • Emergency contact information     • Immunization Records (Oklahoma State Requirements)     • Proof of residency which may include one of the following:  proof of payment of personal income tax or ad valorem taxes; title to residential property; unexpired lease agreement; receipt for payment of rent; proof of payment of utilities; valid motor vehicle operator’s permit; motor vehicle registration or voter registration card. (also see Bartlesville Board of Education Policy FB)     • Home Language Survey     • Title VII (Indian Education Program) Student Eligibility Certification, if applicable     • Internet and other Computer Networks Access and Acceptable Use Agreement/Internet & Media Consent for Release (also see Bartlesville Board of Education Policy ECA)     • Copy of current IEP or 504, if applicable     • OSSAA form for School Activities Eligibility (grades 7-12 athletes only)     • Withdrawal form or final report card from current/last school     • Copy of Oklahoma End of Instruction testing results (if transferring from an Oklahoma high school, grades 9-12) for Algebra I, Biology I, English II, and US History.   3) Working with BPS representative, arrange for an enrollment appointment to complete enrollment process.     • If a student is not 18 years of age, a parent must attend the meeting with the student.  Guardianship must be provided if student does not live with parents and is under the age of 18.     • Information in 2 above must be provided.     • Additional information may be requested at the time of the enrollment appointment.                 If you have any questions or need assistance, please contact Christie Young at (918)336-8600.
